Traci Zimmer is a dedicated Adult/Geriatric Primary Care Nurse Practitioner with a deep commitment to delivering compassionate, high-quality care that prioritizes the unique needs and preferences of every patient. Board-certified by the American Academy of Nurse Practitioners, Traci brings nearly three decades of experience in healthcare to her role, blending clinical expertise with a heartfelt dedication to improving the lives of others.

Traci began her journey in healthcare in 1996 as a registered nurse. Over the years, her passion for patient care and her desire to make a broader impact inspired her to further her education. She earned her nurse practitioner degree from Maryville University, officially transitioning into the role of a provider in 2016. Since then, she has focused on adult and geriatric populations, recognizing the importance of continuity of care and thoughtful communication—especially for older adults managing chronic or complex conditions.

Since 2017, Traci has specialized in primary care, providing services across a variety of settings including patients' homes, assisted living communities, and skilled nursing facilities. Her approach emphasizes collaboration, ensuring that each patient plays an active role in shaping their treatment plan. Traci believes that listening is one of the most powerful tools in medicine, and she takes the time to understand each patient’s goals, challenges, and preferences.

In 2024, Traci expanded her practice to include dermatology care, offering treatment for a range of skin conditions in both clinic-based and long-term care environments. This broadened scope allows her to meet even more of her patients' needs, particularly in populations that may face barriers to specialized dermatologic care.

Traci’s decision to pursue a career in healthcare stemmed from a genuine desire to help people and make a meaningful difference. She finds fulfillment in building lasting relationships with her patients and being a trusted partner in their health journey.

Outside of her professional life, Traci enjoys spending time with her family, shopping, planting flowers, and cooking. Whether she’s relaxing at home or engaging in hobbies, she values the balance that allows her to bring energy and focus to her work each day.
