Complete Health Accountable Care (CHAC) will no longer be directly participating in the Accountable Care Organization Realizing Equity, Access, and Community Health (ACO REACH) Model, effective 12/31/2025. This decision comes after careful consideration and evaluation of our organization’s direction and commitment to providing the highest quality care to our patients, including you.
For 2026, your Complete Health primary care provider has enrolled in Rancho Health Accountable Care Organization, where our provider can continue to offer you the same level of service and benefits that you have enjoyed under CHAC. Your access and level of care will not be affected by this transition. This will be a seamless transition with your care continuing at the same level for 2026.

There will be no change in the way you access your healthcare services from your provider at Complete Health.
As a beneficiary of a participating physician in the ACO REACH program you may have had access to additional Benefit Enhancements or Beneficiary Engagement Incentives implemented by CHAC. With CHAC terminating their ACO participation, those benefit enhancements and benefits engagement incentives will cease to be in effect 90 days after the 12/31/2025 CHAC termination date. Any services furnished under the benefit enhancement and incentives will no longer be covered by Medicare and the Beneficiary may be responsible for the payment of such services.
While we are required to notify our beneficiaries of our decision to terminate CHAC from the ACO model in 2026, your Complete Health provider has joined a new ACO entity for 2026, and you may receive notification from “Rancho ACO,” which means any benefit enhancements or incentives you utilized in 2025 with CHAC will continue in 2026 under our Rancho ACO participation. We want to remind you that these changes will not affect your ability to see your Complete Health provider with no interruption in care.
All other Medicare rights and benefits will remain unchanged through this transition. You can continue to see any healthcare provider, practice, or hospital that accepts Medicare, which includes your providers at Complete Health. Your choice of healthcare providers is always up to you.
