Chronic Care Management

Managing multiple conditions is more than a handful. That’s why we give you a whole team to help.

Don’t handle it alone. Get a dedicated team that checks in between visits to help you manage your conditions with confidence.

What is Chronic Care Management?

Chronic Care Management, or CCM, is a program for patients with two or more ongoing health conditions, such as diabetes or heart disease. It provides you with a support team that offers care coordination and assistance between your regular doctor visits.

Who is eligible for the CCM program?

The program is specifically designed for patients who have two or more chronic health conditions that are expected to last at least a year. If you’re managing multiple conditions, talk to your provider to see if you are eligible for this extra support.

How does the Chronic Care Management team help me?

Your CCM team acts as a constant connection to your healthcare. They proactively call you to check in, help manage your medications, answer health questions, and coordinate with specialists, ensuring your care plan is working for you.

What are the main benefits of the program?

The primary benefits include better control over your chronic conditions, which can lead to fewer hospital visits and a stronger connection with your care team. Patients often feel more confident and less overwhelmed in managing their day-to-day health.

How do I enroll in the program?

Getting started is simple. Just ask your primary care provider about it at your next appointment. They can confirm if you’re eligible and help you enroll so you can begin receiving support.

Does this service replace my regular visits with my doctor?

No, it does not. Chronic Care Management is a supplemental service that works in partnership with your regular doctor visits. It provides extra support between appointments to help you stay on track with the care plan you and your doctor created together.
