Transportation and Social Work

Getting you where you need to be.

Our team helps with life’s practical hurdles, from arranging transportation to your appointments to connecting you with community resources.

How does the transportation assistance work?

If you are having trouble getting to and from your medical appointments, please let us know. Our team can help identify transportation solutions and resources available in your area to ensure you can get the care you need.

Is there a cost for these support services?

There is no cost for a consultation with our team members. Their role is to provide guidance and connect you with resources. Some community services they connect you with may have their own costs, but our team can help you find affordable options.

How do I get connected with these resources?

The best way to start is by talking to anyone on your care team, from the front desk to your doctor or a member of our Patient Advocate Team. Just let us know you have a question or a need, and we will connect you with the right person to help.

What are social work and transportation services?

These are support services designed to help patients with the nonmedical challenges that can affect their health. This includes providing assistance with transportation to appointments and connecting patients with community resources to address social or financial needs.

Why does a primary care practice offer this kind of support?

We offer this support because we believe in caring for the whole person, not just treating symptoms. We understand that nonmedical challenges, like a lack of transportation, are often the biggest barriers to good health. While many practices are not equipped to help with these issues, we built our care model differently on purpose. We see this support as an essential part of delivering truly complete care.

What are some common issues a social worker can help with?

Our team can connect you with resources to help with a wide range of needs, including:

  • Food assistance programs
  • Help with utility bills
  • Access to support groups
  • Questions about housing
  • Advance care planning

Social work support availability may vary by location.
