Your primary care provider, or PCP, is the main doctor for your health. They are your long-term partner, helping you stay healthy, managing chronic conditions, treating illnesses and coordinating your care with specialists.

What is an Annual Wellness Visit?

This is a yearly appointment, covered by Medicare, that focuses entirely on your health goals. It’s not for sick care. Instead, we review your overall health, create a personalized prevention plan and ensure you are up to date on all recommended screenings.

How does the 24/7 Nurse Line work?

When you have a health question, especially after hours, you can call our 24/7 Nurse Line. A registered nurse will listen to your concerns, provide medical advice and help you decide the best next steps, whether that’s home care or seeking immediate attention. Ask your provider if you qualify for this extra support.

Do I need to go somewhere else for lab draws?

No. For your convenience, most of our clinics offer on-site lab services. This means you can often get your blood drawn right here during your visit, saving you an extra trip and helping your provider get your results faster.

Can I schedule appointments online?

Yes. For your convenience, we offer online appointment scheduling through our website. 

Who is on my primary care team?

Your primary care provider leads a team that includes nurse practitioners, physician assistants and medical assistants. You also have access to our Patient Advocate Team and other care managers to ensure all of your health needs are met.

How do I become a new patient?

We would be honored to care for you. The first step is to call the clinic nearest you. Our team will guide you through the simple process of joining our practice.
